I acknowledge that I have read the Official Rules of the Galaxy of Stars® Talent Search and understand and agree to the eligibility requirements and conditions therein. I have answered all questions contained within the Entry Form honestly and accurately and will answer as well all future questions posed to me honestly and accurately. If any information I have provided, or will provide, is found to be false or incomplete, this may be grounds for dismissal of my participation in the Galaxy of Stars® Talent Search and the forfeiture of any and all prizes or awards I may have earned. I understand and will accept all decisions by the producers and/or judges to be final and not subject to challenge or appeal. Further, I understand the producers have no obligation to return or maintain any materials I may provide.

By submitting the Entry Form, I hereby consent to the recording, use and reuse by the producers and any of their respective or prospective assigns, licensees, subsidiaries or affiliated entities and each of the respective sponsors, employees, agents, parents, officers and directors (hereafter referred to as “Releasees”) of my voice, actions, name, appearance and biographical material (hereafter referred to as “Likeness”) in any and all media now known or hereafter devised in connection with the Galaxy of Stars® Talent Search. I agree that the producers of the Galaxy of Stars® Talent Search have the right to use all materials of my likeness presented in videos, photos, CDs, DVDs and all similar related materials I may provide or that may be produced in connection with my participation in the Galaxy of Stars® Talent Search. Additionally, I understand that the Releasees have the right to any photos, video or recordings of any kind that producers will make of me (hereafter referred to as “Materials”) and have the right, without limitation, to use the Materials and my Likeness in all media now known or hereafter devised. Further, I release the producers and Releasees from any and all liability arising out of the use of my Likeness and/or the Materials, and I agree not to make any claim against producers and Releasees as a result of any and all recordings or use of my Likeness and/or Materials.

I agree to defend, indemnify and hold harmless producers and all Releasees from and against any and all claims, actions, losses, damages, liabilities and expenses (including attorney’s fees) incurred by or asserted against producers and Releasees at any time, caused by or resulting from in whole or any part: a) Any use of a prize by winners; b) Any negligent or intentional act or omission of winners with respect to the use of a prize; and c) Any other activity undertaken by or at the request of a winner, pursuant to or in connection with the Galaxy of Stars® Talent Search. Further, I agree to release producers and Releasees from any claim or liability arising out of the Galaxy of Stars® Talent Search, inclusive of the winners use or operation of prizes, inability to use a prize, or any and all other related matters. I understand all winners hereby waive any right they may have, at law or equity, to pursue any claim for damages and that no substitutions or transfer of prizes are permitted.

This agreement shall be governed by and interpreted in accordance with the laws of the State of Nebraska and are applicable to agreements executed and fully carried out within the state of Nebraska. I agree that any and all disputes or controversies arising under or relating to this agreement or any of its terms; any effort by any party to enforce, interpret, construe, rescind, terminate or annul this agreement, or any provision thereof, and any and all disputes or controversies arising under or relating to my participation in the Galaxy of Stars® Talent Search, shall be resolved by binding arbitration and that such proceedings will not be disclosed to any third party and that all information and documents will be treated as confidential, consistent with this provision.

It is understood that at the Galaxy of Stars® Talent Search held at the Nebraska State Fair the Highest Scoring Individual Vocalist in each age division may be offered a management contract with Vision Productions, Inc. Participant is under no obligation to enter into any agreement. If both parties mutually agree to enter into this relationship, the participant will agree that any and all public performances, contracts and career advancement related to or initiated by any promotional services by the Galaxy of Stars® Talent Search, a subsidiary of Vision Productions, Inc., and/or Vision Productions, Inc., during the management contract, requires the authorization of the producers of the Galaxy of Stars® Talent Search and/or Vision Productions, Inc. Further, it is to be understood that the producers of the Galaxy of Stars® Talent Search may freely assign, in whole or in part, any of their rights or obligations contained within this agreement.

By entering my name below and submitting this form, I attest to the understanding, obligation and acceptance of the Official Rules and to the rights, as herein contained, of the producers of the Galaxy of Stars® Talent Search. I declare I am a legal United States Citizen, residing in the state of Nebraska, and that all information entered, as pertains to my age and qualification for entry into the Galaxy of Stars® Talent Search Age Division below is accurate.
